(2) FAX- If changes are minimal, you may fax your proofs. Include only (a) the proof, (b) this form with signature, and (c) a separate list of all changes for clarity, and fax to the number below. NOTE: Do not fax the manuscript.
(3) E-MAIL- If you have 5 or fewer changes, you may e-mail your corrections. Include (a) the manuscript ID given below and (b) a list of your corrections, and e-mail to the address below. NOTE: No TeX-formatted corrections will be accepted.
